About this event

  • Event lasts 14 hours

When was the last time you thought about changing your diet and lifestyle to improve your type 2 diabetes, and overall health? Knowing how to eat well, exercise and manage your medication is important.

This program helps people with insulin-treated type 2 diabetes manage their health. Participants receive a complimentary Continuous Glucose Monitor (CGM).

CGM is a small wearable device which tracks your glucose levels in real-time and sends information to an app or receiver, helping you and your doctor monitor patterns and triggers.

A pharmacist or GP can fit the sensor a week after your session, and it lasts up to 14 days. Each session is a small, friendly group where you can ask questions, meet others, and share experiences.

You will learn how to build healthy habits to manage your diabetes. Topics include:

  • how to eat well
  • managing your sugar levels with and without a CGM
  • recommended exercises and how often
  • why avoiding alcohol is important and tips to help you
  • questions to ask your GP

FREE type 2 diabetes management sessions are for people living with insulin-treated type 2 diabetes who reside in South Western Sydney.

If you would like to attend a diabetes education session but don’t have an email address, text SWSPHN Integration and Priority Populations Coordinator, Alyssa Horgan, at 0402 870 116 with your chosen date.

Organized by

South Western Sydney PHN is a not-for-profit health organisation dedicated to supporting general practitioners, practice nurses and other primary health providers to deliver the best possible care for their patients and improve access to quality local health care for the whole community.

Servicing seven local government areas of Bankstown, Fairfield, Liverpool, Campbelltown, Camden, Wollondilly and Wingecarribee, South Western Sydney PHN is one of 31 primary health networks across Australia aiming to improve the efficiency and effectiveness of health services delivered in our region.

We are doing this by supporting local clinicians; understanding the healthcare needs and service gaps in our community through planning and consultation; and by focusing on improving access to primary care services for patients, particularly those in our community at risk of poor health outcomes.
